If you are nominating a person to receive a Bundle:

Please fill out YOUR information in the first part of the contact form.
You may provide the recipient's situation and details in the message.
Then fill out THE RECIPIENT's information in the last part of the contact form.
Does the recipient have any religious affiliation? (we ask this so we may choose appropriate cards for the recipient.)

We will send you a confirmation and more details via email.

If you are contacting us about volunteering or other information:

You only need to fill out YOUR information and a message. We will get in contact will you within a week.
